They occur through osmosis which is exactly like diffusion except it's in water. As blood passes though the lungs CO2 is released from the blood cell and new O2 is being absorbed and as it passes through other cells just as skin cells, and all the other cells O2 is taken and CO2 is given to the blood cells.
