Yes these facts would concern me.  The mother's vaccine status is very important to the puppies.  They gain maternal antibodies through the milk--if she is not protected then she cannot pass them to her puppies.  We start vaccinating puppies at 6-8 weeks because at this time they are weaned and maternal antibodies can start to wane.
This mother should have received more than just 1 set of vaccines.
The fact that the father has heartworm disease is a concern too.  I would be concerned about any breeder that has not protected her dogs against heartworm disease and is not actively treating an infected dog.  Immune system stress can suppress reproductive function and overall health.  This dog needs to be treated before bred.
I would be also concerned about other genetic issues or husbandry issue that would affect the quality of the puppies and health/development of the puppies.
All reputable German Shepherd Dog breeders should have the hips (and ideally elbows) of both mom and dad OFA (Orthopedic Foundation of America) certified before breeding.  Both hip and elbow dysplasia is common in this breed.  They should make sure they are doing everything they can to not pass this disease on.
Ask to see the kennels and some other puppies too before buying from a breeder.  This will help you assess offspring and quality of the facilities.
Ask your veterinarian or local German Shep rescue group for good breeders in your area.  You can also visit the AKC (American Kennel Club) for information.
