Is There A Pocket Medical History Form That I Can Carry In My Wallet?

I need a condensed form to carry my medical history with me including past surgical history, medications, allergies, etc.

    There is no pocket medical history card for normal peoples. It is used for the peoples who have seizures or dementia.  If you need for yourself, the n you can make yourself and keep it with you. You can put all parameters which you wanted to have on it.

      Yes there is such a thing.
      Your doctor may have one.
      It is a small paper that is held with in a plastic sleeve that fits neatly in your wallet. It can hold information such as medications, allergies, etc.
      You can also store info on your phone, store it under I.C.E.  ( In Case of Emergency )Most medical responders know the code
      you can have it programmed to call your family or you can add text with minimum info.
