That's a really hard thing to do.
The best way to approach it is to tell the person who you are informing to sit down and warn them that you have some "bad" news to tell them.
Be as straight forward but as caring as possible. The person will be in shock and most definitely start to cry. If you are with them then you have the wonderful opportunity to comfort them and be there for them at this awful time.
If you are doing this over the phone... Then the best thing you can do is stay on the line and let the person know you are there for them. They may not be able to talk about what's happened right away, so check on them later and make sure that they haven't harmed themselves.
As someone who has been on both ends of the stick... I can tell you that a patient and understanding ear is the best thing you can offer.
You can bring them food for dinner, offer to help with funeral arrangements... Anything you think will help them.
Having lost someone is so sad... But if you care enough to support that person through this time... They will always remember it.
Good Luck

Tell that death is nothing but separation of soul from body and you will see the departed ones one day when you leave the body.
