Most guide-dogs are trained to hold it in and to only go at a certain time of day, and only in a certain place (at home). The disabled person who uses the dog (usually a blind person) will get someone else to come in and clean the mess up, typically.

The dogs are also trained to give off certain cues (noises, manoeuvres, etc.) if they desperately do need to "go" away from home. The handler will know what these cues mean and take the dog somewhere appropriate. Again, the blind person may need to enlist someone else's aid to dispose of the waste satisfactorily.

Other disabled people who use guide dogs (deaf, etc.) often can deal with the waste themselves, anyway.
