This is often difficult, especially for subjects like literature or foreign languages where you may not have a clear framework. It's much easier if you plan the whole essay before you start, then the introduction will often arise naturally. Planning need only take a few minutes and will save lots of time later. Just get your basic ideas – about six will do for most exam essays – and make a quick note for each. Then look at the question again – are they all relevant? If so, now see how they fit together. Hopefully your main argument or topic will now be clear, and the introduction can just state what it is.
A simpler way to start is to take an idea from the question and analyse it (don't echo the words, though.) Questioning it can be effective, eg, "In order to answer this queston, we must first define what is meant by..."
