Sometime there is not obvious openings into the skin it could be a little bug bite.  You don't know how many times I see people come in the ER with cellulitis which is a skin infection and we can not see an opening to in the skin and the patient can't remember any injury.

Germs and infections enter your body through the orifices in your body ie your mouth from your hands, that is why it is suggested that hand washing is essential to get rid of or at least curtail some of them look at how many things we touch during the course of one day ie money, door knobs, pens in a bank or office to sign papers, and many more items and things that can be unclean, the germs need a way to exit your body and therefore they exit through the pores of your skin and no there does not have to be a break in the skin it lodges in one area and causes a infection in the way of a bump, boil or other types of lump to show up on the outside skin, there are times when they have to be treated medically....the best to you
