Yeah these days it is usually associated with stress.
The phrase literally means there is too much for something to handle therefore 1 part of it must eventually break.
This can be a reference to someone having a heavy work load, family life and additional stressors on them or a pipe with steam in getting higher and higher pressure.

To me it means changes have to be made as far as the way things are going at that particular time to help make things work better in your life for you and you can make a change.
