There are several organisations that will arrange this for you - flypets is one example - but they aren't cheap, and it would be better to contact the airline direct. They tend to have different animal transportation policies, and costs vary too (you will almost certainly have to pay something.) Vaccinations etc may be an issue too. Most airlines have a section on their website for pet policy, so you may be able to find the information there.
