    Affidavit means a written declaration under oath made before a notary public. An affidavit of service is a common verification. It verifies to the court that the complaint and the summons have been sent to the concerned parties. Though it is a routine matter, it is required by the law. The document is necessary to prove the court that the defendant has received the documents issued on its name. It is usually filed after the defendant receives the summons.

    The affidavit of service is also known as CAO 16-14. The document requires the plaintiff's name, address and telephone number along with some other details such as case number. The procedure to file an affidavit of service in Idaho is same as other states.
