Can A German Shepherd Dog Only Have One Puppy?

It's possible but unusual... I'd have her checked out by a vet just to make sure all the pups and placenta's have been delivered, to be on the safe side.

It's possible that there is only one, but this would be very rare, typically there is anywhere from 3 to 8 puppies in a litter. She could have still pups in her belly yet, or she could have a delayed delivery, it's been known that a dog delivering for the first time, have taken a couple of days to deliver all of the pups.
I would get her to the vet to make sure there isn't something more serious going on with her. Hope this helps, good luck.
