Database design can refer to two things.
1) Designing GUI for database driven applications, these can be web based but are usually locally operated so you more than likely need point 2.
2) Designing websites that integrate with databases such as MySQL that dynamically populate with data. Usually you would be required to work with a programming team who would take your design and implement the database into it.
Common usage includes e-commerce stores, CMS and general social communities as well as any more complex site deployed by a design agency
