It depends on what has occurred.  If it was a grave transgression that you just can't shake the bad feeling too and the hairs on the back of your neck stand up...then no, go with the gut feeling.  Walk away from it and think for a long while, let the anger and hurt fade a bit.  Logical thinking does come until the storm is over.  If still the feeling doesn't go away and unforgivable in your book then no.  

Nobody can step comfortably around one another when the problem still pops up in your mind.  Some people you must release in life and walk away from.  But if you feel in your heart that you can see past the problem and you truly in your heart believe that you can try again, then do so but this must be done only with the true openness of heart and understanding that you are cleaning the slate.  One can forgive and forget, some can forgive and not forget but learn.  It is truly up to you in giving the second chance.

If the result is a negative one, you need to asses consequences & give another chance

It depends on what that person has done. If it is something very serious like cheating on you, then he is very likely to do it again. If you still have feelings for that person, you should let him/her know how exactly you feel and how you will not stand being taken for granted again. If it is something minor, then you should forgive and forget.
