Short forms, recognized sometimes as computer certifications, are not generally available, but are cheaper than photocopies and a great deal more easily available. Limited information is in use from the unique birth record (the long form) and stored in a file that can be accessed rapidly when birth certificates are wanted in a small amount of time.

The short form characteristically includes the child's name, date of birth, gender, and position of birth, though some also comprise the names of the child's parents.
