    Short forms, recognized sometimes as computer certifications, are not generally available, but are cheaper than photocopies and a great deal more easily available. Limited information is in use from the unique birth record (the long form) and stored in a file that can be accessed rapidly when birth certificates are wanted in a small amount of time.

    The short form characteristically includes the child's name, date of birth, gender, and position of birth, though some also comprise the names of the child's parents. When the certification does comprise the names of the parents, it can be second-hand in lieu of an extended form birth certificate in approximately all circumstances. Nearly all states in the U.S. subject short forms certifications, on in cooperation condition and local levels.

    In the U.K., there is no real "long form", in the intelligence that the Registrar General place of work does not in fact keep a human being certificate on file for every birth. Every one birth certifications subjected from a U.K. registrar is based on birth particulars taken from a birth record. Typically, the Registrar General will issue a documentation that comprised the name, gender, time of birth, and position of birth of the child.
