How Do I Write A Letter To My Insurance Company To Appeal A Medical Bill?

I had a procedure done at my OBGYN office and before it was done I had the Dr. Office call my insurance company to make sure it would be covered, well my Dr office stated that my insurance company would cover 100%. A few weeks later I got a bill from my Dr. Office for the full amount of the procedure, I called my insurance company and they then told me that they would not cover it. I was told by my ins. Company that my dr. Office an I had to write a letter to appeal.
