In Kentucky, and I assume most states, you need to apply to the Office of Vital Statistics. You can apply online or you may also go to your local Health Dept. And pick up an application to send to the Vital Statistics Office .

Actually most states now require a reason for you to get a birth certificate.  You are allowed to get one for yourself, your child or your deceased parent, or most deceased close relatives.  More and more though, the days of getting a copy of anyone's birth or death certificates are a thing of the past.  And make sure you specify if you want an "official" or "certified" copy or just a plain copy.  There will be a difference in cost.  Generally applying at the state level as opposed to the county level will take longer, depending on the population of the state.  Trying to get one from the State of California Dept of Vital Statistics took over 6 months for my brother, but when he applied at Kern County he got it in 3 weeks.  When I applied for mine at the Oklahoma County Health Dept, it took only a matter of minutes.  Definitely go the local route if that is available to you.
