An affidavit is a written legal sworn statement testifying that a certain facts or set of facts is true. In your case, you will likely be asked to swear to the truth that you and your partner are in fact eligible to be considered as common law married, according to living arrangements, relationship, and even perhaps the length of time you have been together. It is often required to award medical or legal benefits and to prevent fraudulent use or assignment of such benefits as is allowed to those legally married as they would receive under law.
