Sample Of Appeal Letter To Health Insurance After Drop Of Coverage?
Am a farmer, was dropped from my medical health insurance company for pre-existing conditions; a year ago under a different insurance agency my doctor suggested I take a test for a sleeping disorder that turned out to be unnecessary. I am now dropped from new insurance company - how should appeal letter be written?

I am in need of an appeals letter of none payment and I am on a leave of absence due to a major back surgery. I have until May 14 to go back to the doctor office to see if I am able to be released to go back to work. I was unable to pay on circumstances as being on disability. I am going to borrow the money to be able to pay it in full. Can you help me with this letter?
