To Add My Husband As Joint Tenants To Property (currently In My Sole Name) Which Form Do I Use?

First I should ask do you both have a will?  Without it the state will automatically make one for you, which is much more expensive for the estate.  Next definitely add him to the property unless you don't like him, then the added gift will be okay.  

But if you really love one another you should use the will only for making your final arrangements, (instructions for executor).  Then put your main assets into a trust, with a pour over clause added to be put the small stuff into the trust upon your death.   With a trust  your assets do not have to go to probate court, the executor can execute the trust himself.

The form required if you want to make your husband as the joint tenant of a property is known with name of the function that it performs; it is called 'Joint Tenancy Application Form'.
     
The process to fill the form is also very easy and even you can fill it online to make it move for the initial processing. After submitting it online you should keep a signed copy also. When you are submitting the form you should also print a copy for further process. When you take the printed copy you will have to sign it and then will have to send it to the address that was given at the place when you are signing the application.
