During middle age, the skin around the eyes starts to lose its elasticity. The muscle that holds the eyelids in place thins out and weakens, and as collagen is lost in the dermis wrinkles appear on the parts of the skin that moves the most. The eyelid thins and stretches under the influence of gravity, and the action of blinking and smiling plus environmental factors, such as smoking and sun exposure, all serve to make the eyelids sag and wrinkle.
In some people as they get older, the top eyelid can droop so much, it actually hangs down and covers up their view of the outside world. Some people who have an eyelid operation do so to make it easier to see, rather than achieve a cosmetic improvement. This operation could be justified as being necessary.
When the eyelids have drooped and just look unsightly, then the operation is more cosmetic and it is often done in conjunction with a general face lift or brow lift to achieve a younger looking face. This is not medically necessary but people will pay large sums of money and go through a painful operation to achieve a better appearance.
