You can't control your boss's understanding. The only way to go about this is directly. Ask for a meeting with your boss in a letter, stating briefly what your issues are and that you need some compassionate assistance. CC'd (also send) it to the Head of Human Resources too, that will help if things get messy.

Most business practice does not include compassion, money isn't made compassionately. However, most modern businesses understand that people have problems that do interfere with their working lives and that paying attention to these issues will help the employee resolve them and also bring them back to work feeling supported and assisted. This further builds the relationship between manager and employee and employee loyalty.

If you don't get sympathy from the boss, it may be worth asking for help from Human Resources, or perhaps seeking help from outside sources to negotiate with the boss.
