Really the only way is to be your own boss – in other words, give yourself orders! Everyone is different, but what is true for everyone is that self-discipline gets easier with practice. After a few weeks, sitting down to your desk or other workstation at a certain time feels quite natural – you may even feel a bit worried if you start late.
It also helps to be very specific about allowing yourself rewards. Plan breaks in advance rather than just taking one when you feel like it (otherwise you will probably "feel like" about 5 every hour) and really take them. Get up from your desk, go and do something nice. I'd recommend a proper lunch break too – you can do a lot more in the afternoon if you've had a little rest.
Finally, it helps to break the routine sometimes. Why not plan a half-day off now and then, and make up the time by working longer the rest of the week. Or have an occasional long weekend – that's the best of working from home.
