    How Do I Get Birth Certificate For Myself And My Daughter. I Was Born In Michigan And She Was Born In Memphis Tn?

    Our counties and states have a Health Department where birth certificates are recorded. Check with your local Health Department in the city or town you are in now. They will be able to give you a form for you and for your daughter and the mailing address of the State Health Department in both Michigan and Tennessee.

    I my state the fee for the birth certificate is $10.00 which is sent with the request form. You will probably be required to send a similar fee with each of your requests.

    You will need to know the name of the county in the state where each of you were born, if you are certain, the staff at your local Health Department shold be able to help.
