An application in person has to be made when one is applying for a US passport for the first time. Documents required include an application form (available at travel.state.gov) proof of citizenship which can include a previous passport, birth certificate, Naturalization Certificate, Consular Certificate of Birth, Driver's License, Military ID card, Two recent passport size photographs, application fee and social security number. The Department of State's Bureau of Consular Affairs under its Office of Passport/Customer Services has designated several post offices, public libraries, clerks of court, and county, state and municipal offices to accept passport applications. A list of centres that accept these applications can be accessed from the web site mentioned above. Most of these centres are willing to accept applications by appointment only.
