Astronauts need to exercise because their bodies don't get a lot of the natural workout that gravity gives them on earth. Their hearts don't need to work as hard to pump blood around their bodies in a zero-gravity environment, so by exercising they make sure that there's no deterioration of the heart muscle, or any of their other muscles, for that matter.
