My cat has always been good about using the litter box and just recently she has starting urinating on the floor just outside of the litter box.

This is common in cats. It is indicating behavioral problem. There can be many reasons behind it. This can be a reaction of its anger. May be it is unhappy and uncomfortable with the box. May be it's is acting like this because of some urinary infection that is making it uncomfortable.
