What Causes Cat Annal Gland Problems?

I had my cats annal glands cleaned out 2 weeks ago. Because she was pulling her hair out around her tail and on her back. She is still doing it. I have had the vet treat her for fleas also and I don't think that is the problem. Is her diet causing the annal problem?
