What Could It Mean If My Period Is Irregular And Extremely Painful With Alot Of Clotting?

I have an IUD and I have had mine for 7 months. I have irregular periods with alot of extreme cramps and huge clots as well. I thought this was normal. Well every month I would feel pregnant, with symptoms of be naseated, swollen painful breasts, but then I would have my period. So I wouldn't think nothing of it. I had noticed I have been gaining wieght about a month ago so I started dieting and taking fish oil. Then I got really bloated looking and feeling. So I read and fish oil can cause that so I quit taking that. 3 weeks ago. I had another period, but I started feeling movment about 2 weeks ago. It's twitching, somtimes it's 6 or 7 nudges in the same spot and sometimes pulsing. I took a test and it was negative. I don't know what is going on, but I have a dr's appointment in 7 days.
