In a normal tower pc the modem is located inside the machine. It comes in the form of a 'card'. The ports on the card points out of one of the slots at the back of the machine so that you plug a phone line in.
Modems can be replaced quite easily by uninstalling the drivers and then removing the modem card from the PC. If you're unsure about these procedures it's best to call a technician that will be able to take your modem out and put a new one in.
Laptops on the other hand commonly come with wireless modems.
