This depends on the car.
On some cars you open up the engine computer and replace a ROM chip. If the chip is in a socket this is easy, if the chip is soldered in this can be difficult.
On some cars the "Module" is simply a replacement engine computer that already has modifications made.
On some cars you can use a programmer to load a new program into your existing engine computer.
So it all depends on what car you have. In general though this modification is one of the cheapest ways of getting better performance.
