The first actual president of the fledgeling USA was George Washington, from 1789-1797.
The habit of addressing the American President as Mr. President probably originated with the British habit of addressing the Speaker of the House of Commons as "Mr. Speaker".
Americans would have, in true providential fashion, have always referred to their national leader as "The President". After all (see above blurtit answer about origin of the term), unless they had attended University themselves they would never encounter the term in any other context.
The same can be said pretty much of any country which adopted the notion of a "President" as its national leader.
