If you have lost your national insurance number card, the first step is to report the loss the relevant authority. You can tell either the Social Security office, or the National Insurance Contributions Office.
Possession of a national insurance card is not strictly necessary: so long as you know your national insurance number you should be fine. However, if you would like a replacement you should contact one of the offices above, who will provide you with an application form. Having filled this in you should return it to the same authority, who will issue you with a replacement. You may have to pay a fee, and it should be noted that you are only permitted one replacement national insurance number card. So look after it!
