Answer Question

That is a very sick puppy who may have bad tapeworms. The vomiting and diarrhea will cause severe dehydration, which is not only incredible painful to a dog ( it gives them a severe headache ) but also deadly. You need to take this animal to a vet immediately.

Take this dog to the emergency clinic as soon as possible.  Puppies do not have a lot of reserve and when they get sick the secondary complications--dehydration, hypoglycemia, and hypothermia--can become serious and fatal quickly.
Intestinal parasites may be the primary problem or may be a secondary complication.  There are several different de-worming medications and you may not have been using the correct one.  It would be best to have a fecal sample examined by your veterinarian to determine the type of intestinal parasites involved and start appropriate therapy.
Vomiting can be seen with severe intestinal parasitism but is not common.  I have concerned the intestinal parasites are not the main problem.  Parvo virus is a huge concern in any puppy presenting with these signs-especially if the puppy has not been vaccinated.  This virus attacks the lining of the intestines and causes systemic bacterial infection.  Without treatment it is usually fatal.
Other causes may be foreign body obstruction (also potentially fatal), toxin exposure, and dietary indiscretion.
I would have your puppy examined as soon as possible for diagnosis and supportive care.
