You disc could be scratched or you system may need cleaning. If the disc is scratched you can polish it with car polish and it will be like new,or you can buy one of those kits to fix damaged disc.

Most likely the lens need's cleaning , to fix that you need to take it to a specialist or take it apart and clean the lens your self ( careful not to loose any parts ) .
