I believe the best form of documentation while an interview is being held is sound recording or video recording, if possible. The interviewer should only make notes of specific points on which he/she can build upon the interview itself. This is because, this way the interviewer can focus on the interview itself and getting more related information. In traditional handwritten documentation, the focus of the interviewer is on getting exact information copied. So this way, he can later get the complete and exact manuscript of the interview in written form through the sound recording.
