I found 135 different hosting sites that support PHP and MySQL database at www.free-webhosts.com, though I can't recommend any of them.  They do have a search feature that allows you to input the requirements that you are looking for that may narrow down the field a bit more for you and to help you find what you're looking for in terms of space, traffic, etc.  

Unfortunately, the three free website/hosting sites (geocities, angelfire, and tripod) that I am most familiar with don't appear to meet your requirements for PHP and MySQL.  Geocities has it included in their plan for $10/month.

I don't think any of those websites really mean what they say. If they give you free hosting, you get a sub-domain, or you have to pay for the domain name.
