A Form W-9 is an application for a federal Tax Identification Number - a Social Security or Employer Identification Number. If you are employed, you must have a Social Security Number (SSN). If you are in business and pay employees, you must have an Employer Identification Number (EIN). It doesn't have anything to do with the amount of taxes you pay or what state you live in.
