My early signs were a cough that wouldn't go away. It was a productive cough which was a sign my lungs were filling with fluid. My doctor thought I had a pneumonia and after 10 days of antibiotics I could hardly breathe and had no strength. After a trip to the emergency room. They told me I had a heart attack. They put in 2 stents and that helped. So now when I look back the first 2 signs were the persistent cough and the shortness of breath.
