The baby is connected via the umbilical cord to the placenta. THe placenta is large , red disk with finger like projections known as villi which efficiently fit into the wall of the uterus. On one side of the placenta there are large spaces with the mothers blood on the other are large spaces with fetus blood because the walls of the placenta are very thin this efficiently allows of the diffusion of substances across from the  mothers blood to the fetus and vice versa. These substances may include  cocaine.
