I have had to go in on scheduled vacation days and lost a PTO day because of it. HR fixed it though no biggie.

It depends on whether you had a leave of absence and your vac was pro rated; or it may be an HR error. Or if you filled out your vac form and then decided that you were going to work and not take it then you need to let someone know what happened so that they can correct it. On the other hand if your time was up already and it happened to be on that day that you worked because you forgot about it then you just plain lost it .
