If it is someone you know well, I don't think you need their address or your return address at the beginning. They will know by your signature. In this case a simple, "Your generosity is so appreciated. I can't tell you how grateful I am. Thank you," and your signature.
If on the other hand it is to be a more formal letter for someone you do not know very well, the person's name and address should be at the beginning. I would keep the message the same. If someone has helped you, you are very good to send a thank you note. It will be appreciated and is the right thing to do.
