To list your telephone number, first you have to contact your local telephone exchange and give them your complete residential or work address. You also have to politely enquire whether they could send one of their representatives to the address which has been specified by you at a time which is convenient to you.

If they reply in the affirmative and send one of their representatives to the address which has been specified by you at a time which is convenient to you, they will hand you over a form which will contain such details as your name, address, telephone numbers for contact, etc.

When you have filled this form, you have to hand it over the representative of your local telephone exchange. He will return to the telephone exchange and hand it over to the concerned official, who will verify your details and then include it as one of the entries in the next year's edition of the telephone directory.
