Qualm is having a sudden feeling of sickness. It could even be something like faintness or nausea. This feeling is generally very disturbing. It is a feeling that is very unpleasant and is generally felt just before you can actually feel something is going to be wrong. It is a very mild state of nausea.
It is a very disturbing feeling of a self doubt. Like W. H Hudson has stated "an ignorant ruffianly gaucho, who . . . would . . . fight, steal, and do other naughty things without a qualm".

Qualm is a sudden attack of illness, faintness, or nausea. Also, a sudden access of usually disturbing emotion(as doubt or fear). It could also be a feeling of uneasiness about a point, especially of conscience or propriety.
