Whom Do I Make A Report To If The Report Is On Cps?

I live in arizona. I had visitation for 2 hours at the cps office. Every toy was filthy dirty, every riding toy was broken, inside one of the 4 toy boxes was individual staples(about50) 1/4 inch plastic tube and other items that any small child could choke on. I said something that day. But i want to write or file an official report. Where do i do this?
