Wow on bickles' answer. Sounds like you have a pinched sciatic nerve.Usually caused by one or more ruptured discs in the lower lumbar region of your spine. You need to see a neurologist or an orthopedic doctor. You can then file for temporary disability until you are well enough to go back to work. The doctor will then fill out the necessary papers once you file.

Were you injured three times? You'll have a tough time making a claim unless you have one documented incident and a date of injury. A doctor will have to write you a note taking you off work, if this date of injury can be established.
If you can't prove a trace to a specific injury, don't file a claim. Workers compensation insurance companies take fraud very seriously. Consider making a disability claim instead.

Yes, any injury that can be shown or proved by testing from a doctor or specialist should be approved for disability. It may only be temporary disability until either treatment or a surgery can be preformed though. Believe it or not i had to have my boss and a lawyer to actually argue with the arbitraitor for the workmans compensation department so i could get compensation. You might also think about filing for partial disability so you don't have to lift anything heavy anymore ,because they will try to send you back to work.
