You need to hook the radio outs (to the speakers) into the amp inputs and the amp out into the sub. 

It really depends on condition and the buyer.  I have seen these sell from $500 to $1000+.  These are VERY hard to find in really good shape, so that's the key factor.  If you're holding a NM you could fetch 1200.
