My thoughts would be because if use are using it for a plantar's wart or something on the feet it could result in a small wound first and a small wound on the feet of a diabetic or someone with poor circulation could become a more troublesome wound. Talk to your doctor about it. They will know for sure. Maybe even a pharmacist could answer this question as well.

Precisely---the last thing a diabetic needs is a wound on his or her foot.

I feel so bad for diabetics. I have gestational diabetes when I was pregnant with my son. It really does change your life. I couldn't wait to give birth just so it would go away! 😔
Sad sad sad.

As Yin and Yang suggested:

(For diabetics), the greatest concern is with salicylic acid, which acts to intentionally create a wound..... Since hyperglycemia inhibits granulocyte activity and lymphocyte function.

Another doctor of his prescribed a medicine that he was suppose to take one then gradually increase it to two. I started observing he had tachycardia and something wasn't right. I pointed it out to his Dr. Sure enough the rapid heartbeat stopped happening when he took it back down to one.
